29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction 6 feet (1.8 m) or more above lower levels were not protected from falling by the use of guardrail systems, safety net systems or personal fall arrest systems nor was an alternative fall protection plan which meets the requirements of 1926.502(k) developed and implemented: On or about January 28, 2019, and at times prior thereto, employees were setting trusses on the third floor of building #5 of the apartment complex under construction while an employee was standing on the top plate approximately 9 feet high receiving a truss from the rough terrain forklift with no fall protection, exposing employees to a fall hazard.

29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided: On or about January 28, 2019, and at times prior thereto, employees were setting trusses on the third floor of building #5 of the apartment complex under construction while an employee was accessing the top plate of the inside wall by climbing up with no ladder approximately 9 feet high, exposing employees to a fall hazard.
