Specially, the electrical control box door was missing a lock on the panel door leaving two openings. Employees were exposed to an electrical hazard.

29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective: JOB SITE: 665 Gravois Bluffs Road, Fenton, MO 63026, on or about 1/29/2019, in the break room, employees were permitted to use a food vending machine with a missing a ground prong on the power cord. As a result, the path to ground from the equipment was not permanent, continuous, and effective closed. Workers were exposed to an electrical hazard.
