29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws: On or about January 31, 2019 in the chemical mix room above the mixing station platform employees were exposed to electrical hazards where a drop cord and plug from an overhead, four inch metal electrical box was not provided strain relief.
