29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury: On or about February 6, 2019, and at times prior thereto, an employee was exposed to a fall hazard of greater than 6 feet to the ground below while installing plastic panels to a metal roof without hazard recognition to identify that the fall protection used, a Rebel Protecta self-retracting lifeline, was not manufactured and designed to be used on a metal roof.
