29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:  A)     Employees were found to be overexposed to noise in the various remote pump houses located throughout the Irrigation system.  One employee was exposed to a noise level of 93.2 dBA over an 8 hour period, another was exposed to 89.8 dBA.  The employer did not have a hearing conservation program in place.

29 CFR 1910.95(g)(6): At least annually after obtaining the baseline audiogram, the employer did not obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels :  A)     Employees working at remote pump houses throughout the irrigation district, were exposed to noise levels above the permissible exposure limit.  One employee was exposed to a noise level of 93.2 dBA over an 8 hour period, and another employee was exposed to a noise level of 89.8. The employer did not have audiograms for these employees within the last 12 months.

29 CFR 1910.253(b)(2)(ii): Inside of buildings, cylinders were not stored in a well-protected, well-ventilated, dry location, at least 20 feet (6.1 m) from highly combustible materials such as oil or excelsior:  A)     In the welding portion of the maintenance building, oxygen, and acetylene bottles were stored out on the floor with nothing in place to prevent them from falling over, or being knocked over by employees, fork lifts or materials being fabricated on nearby welding tables.  Some of the bottles that were stored out on the floor did not have caps on.

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet:  A)     In the welding shop and area immediately outside the shop oxygen and acetylene bottles were stored  next to each other.
