29 CFR 1910.178(l)(1)(i): The employer shall ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l). At the establishment, in the shipping department, employees operating a Toyota 4000 pound forklift, serial number 78430, model number 75GOU20 were not trained by the employer in the safe operation of the forklift, exposing employees to the hazard of being struck by the lift.
