29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. a) Jobsite - Tin Slot Line, Water Tank, Auger - Employees were exposed to entanglement and amputation hazards while processing Shot with an auger that was not guarded. c) Jobsite - Double Stroke Solid Die Cold Header Machines, rotating discs feeding line of raw material - Employees processing Anodes were exposed to ingoing nip point hazards from the rotating discs that were unguarded. On or about 1/31/18.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i). Jobsite: Electrical panel boxes behind shipping, 120/208 V Panel Box (second from overhead door) - The energized panel box was missing blanks for breakers #7, 9, 11, and 18 exposing employees to shock hazards. Observed on or about 1/31/18.
