29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) Mountain Construction, LLC at 2956 Sykes Dr. Ft. Collins, CO 80524: On or before 1/31/19 the correcting, creating and exposing employer did not protect employees from injury in that employees were working on the roof without fall protection. An employee was unloading a rough terrain forklift on the roof of a two story single family home without wearing fall protection. This condition exposed employees to a fall hazard of 20 feet.

29 CFR 1926.1053(b)(13): The top or top step of a stepladder was used as a step: (a) Mountain Construction, LLC at 2956 Sykes Dr. Ft. Collins, CO 80524: On or before 1/31/19 the correcting, creating and exposing employer did not protect employees from injury in that employees used a step ladder to climb onto the roof joists to access the roof. The top step and the top of the step ladder were used to gain enough height before climbing at least two feet to the roof joists. This condition exposed employees to an unsafe ladder.
