29 CFR 1910.147(c)(2)(ii): The employer's tagout energy control program procedures did not comply with 29 CFR 1910.147(c)(3) and no lockout was provided for employee protection. a) 800 Van Reed Road, Wyomissing, PA: On or about January 25, 2019, a temporary employee performing cleaning activities on "line 5" was exposed to contact with moving machine parts when the conveyor belt was reenergized. Lockout/Tagout devices were removed prior to this cleaning activity. The emergency stop button was activated to shut down the conveyor. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1910.147(f)(1): When lockout or tagout devices were temporarily removed from the energy isolating device and the machine or equipment was energized to test or position the machine, equipment or component thereof, the sequence of actions were not followed in accordance with 29 CFR 1910.147(f)(1)(i)-(v): a) 800 Van Reed Road, Wyomissing, PA: On or about January 25, 2019, a temporary employee performing cleaning activities on "line 5" was exposed to contact with moving machine parts when the conveyor belt was reenergized to determine if cleaning was effective. Energy control procedures were not used prior to the additional cleaning. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.
