29 CFR 1910.36(h)(2): Outdoor exit route was not covered where snow or ice was likely to accumulate.  (Exception: Not required where employer can demonstrate that snow or ice will be removed before it can present a hazard)  A.  On or about February 4, 2019, In the following locations, snow and ice had been allowed to accumulate in an exit route:  1.  Receiving Door:  The emergency exit route outside the identified EXIT door had not been cleared of ice or snow from a previous snow storm.  The accumulation did not allow for the door to open fully.  The walkway to the lot had not been shoveled or sanded.  2.  Exit door at Bay #4:  The emergency exit door led to a snow- and ice-covered landing and four stairs that led to a parking lot that had not been cleared near the building.

29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered:  A.  On or about February 4, 2019, Building 4 Warehouse:  The Tailift propane powered forklift was operated with 60" forks, which would have a 30" load center.  The load chart on the vehicle listed only data for a 24" load center.

29 CFR 1910.305(g)(1)(iii): Flexible cords and cables were used for purposes prohibited by subparagraphs (A) through (E) of this paragraph:   A.  On or about February 4, 2019, Bay 1:  An orange flexible extension cord was run from a wall outlet inside the building through the doorway to the outside of the building where it powered an engine heater on a diesel truck operated by the company.  The cord was subject to damage from being closed in the door, and employees exposed to electric shock hazards.
