29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. a) 78th Security Forces Squadron - On or about December 27, 2018, at Gate 14, Warner Robins AFB, Georgia an employee was struck by a POV resulting in the amputation and hospitalization of an employee. The employer failed to report the incident to OSHA within 24 hours. The report was made on 01/18/2019.
