29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person (i.e., a person who is capable of identifying existing and predictable hazards in the surroundings or working conditions which are unsanitary, hazardous, or dangerous to employees, and who has the authority to take prompt corrective measures to eliminate them):    (a) SML Construction LLC, at 131 Sage Drive, Brighton, CO: On 2/1/19, the employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person (i.e., a person who is capable of identifying existing and predictable hazards in the surroundings or working conditions which are unsanitary, hazardous, or dangerous to employees, and who has the authority to take prompt corrective measures to eliminate them).  Employees were removing and installing siding of two story townhomes.  Employees performed this work while on the roofs of the first stories and second stories of the townhomes.  Employees utilized ladders to access upper levels.  The employer did not inspect the worksite to determine hazards and implement appropriate corrective action.  This condition exposed the employees to approximate 10 foot and 12.8 foot fall hazards.

Employees were removing and installing siding of two story townhomes.  Employees performed this work while on the roofs of the first stories and second stories of the townhomes.  The employees utilized an aluminum extension ladder to access the second story roof from the first story roof.  The ladder side rails extended approximately 1.5 feet above the upper landing surface.  This condition exposed the employees to an approximate 10 foot fall hazard.

Employees were removing and installing siding of two story townhomes while working on sloped roofs.  The  employees placed aluminum step ladders onto the lower sloped roofs to access the upper roofs, without securing the bases of the ladders.  This condition exposed the employees to approximate 12.8 foot fall hazards.
