29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy:   815 E. Cherry St, Troy: Employer is exposing employees to hazardous conditions.  The employees are permitted to perform maintenance activities on machines without machine specific procedures in place.   On 02/21/2019 lockout/tagout activities were performed on CNC HAAS mill without machine specific procedures in place.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed:    815 E. Cherry St., Troy, Northwest of the warehouse: Employer is exposing employees to hazardous conditions.  The employees work by an electrical panel with exposed unused holes.   On 01/31/2019 an electrical panel on the main aisle had two knockouts missing, exposing employees to electrical hazards.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:   815 E. Cherry St., Troy Warehouse: Employer is exposing employees to hazardous conditions.  The employees are permitted to work without being informed of the presence of a new hazardous chemical in their workplace.   On 01/31/2019 it was determined that employees who were present in the warehouse during painting activities were not informed of the identity and the hazards of the new epoxy paints used for remodeling activities.
