29 CFR 1910.119(e)(5): The employer did not establish a system to promptly address the team's findings and recommendations; assure that the recommendations were resolved in a timely manner and that the resolution was documented; document what actions were to be taken; complete actions as soon as possible; develop a written schedule of when these actions were to be completed; communicate the actions to operating, maintenance and other employees whose work assignments were in the process and who may have been affected by the recommendations or actions:     a) Throughout the Ammonia Refrigeration System - On or about January 31, 2019 and times prior to, the employer had not ensured that the recommendations from the 2015 process hazard analysis were resolved in a timely manner and that actions were completed as soon as possible, thus resulting in a chemical hazard.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a) Marination, Injector Lines - On or about January 29, 2019 and times prior to, the rotary filters were not adequately guarded, thus exposing employees to caught-in hazards from the rotating auger blade within the rotary filters.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):    a) Boiler Room #1 - On or about January 30, 2019, live electrical parts were exposed when employees opened the control panel to reset the low water button for the 300hp Industrial Boiler Co. boiler, thus exposing employees to electrical hazards.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws:    a) Portioning (DSI/Blue Room) - On or about January 30, 2019, the outer sheathing on the flexible conduit that energized the hopper motor on the long blue infeed conveyor line was pulled away from its connection point on the wire housing, thus exposing employees to electrical hazards.
