29 CFR 1910.134(k): The employer did not provide comprehensive, understandable training which did not occur annually and/or more often if necessary: a). On or about 2/4/19, on site, for the employee that voluntarily dons an e3000 PAPR respirator on a daily basis while conducting welding tasks, the employer did not provide training. b). On or about 2/4/19, on site, for employees that voluntarily don a 3M N95 #8511 respirator during welding tasks. The employer did not provide training as outlined in Appendix D of the standard.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: a). On or about 2/4/19, on site, for employees that use D-Spat on a monthly basis. D-Spat contains chemicals, such as, but not limited to, Potassium Hydroxide. The employer did not implement training.
