29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):    Instance 1: On or about 1/31/2019, at 3304 63rd Square, Vero Beach, Florida, an employee fell approximately 9ft from a residential 6:12 pitch roof while installing plastic caps on the peel-n-stick due to the lack of fall protection equipment.    Instance 2: On or about 1/31/2019, at 3304 63rd Square, Vero Beach, Florida, employees performing roofing activity on a 3:12 pitch roof were exposed to a fall hazard of 9ft or more due to the lack of fall protection equipment.    PLEASE NOTE: CERTIFICATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.

29 CFR 1926.502(d)(6)(iii): A snaphook not of the locking type was engaged to a Dee-ring to which another snaphook or other connector is attached:    On or about 1/31/2019, at 3304 63rd Square, Vero Beach, Florida, two employees performing roofing activity were exposed to a fall hazard of 9ft or more due to attaching their snaphooks of the safety rope to the same Dee-ring on the roof.
