29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. a. On or about January 24 2019 at 7433 Emerald Dunes Drive, Suite 200 Orlando, FL 33822, an employee suffered a work related injury to his right foot from being struck by a forklift at 4:30 AM January 24, 2019. The employer was aware of that day that the employee was hospitalized. The employer notified OSHA on January 25th at 3:10 PM of the work related hospitalization.
