29 CFR 1910.36(e)(1): A side-hinged door must be used to connect any room to an exit route:    On or about February 4, 2019, and times prior thereto, the employer failed to ensure a door used for an exit was side hinged. The employer is using a roll-up garage door for an exit door in the rear of the facility. Employees exposed to hazards such as, but not limited to asphyxiation and burns from a fire resulting from a delay in exiting the building.

29 CFR 1910.36(g)(2): Exit access were not at least 28 inches (71.1 cm) wide at all points:    On or about February 4, 2019, and times prior thereto, the employer failed to ensure the path to the exit was at least 28 inches wide. The exit access to the back door was measured at approximately 18 inches. Machinery was installed that limited the access to the exit door. Employees exposed to the hazards of an asphyxiation and burns from a fire.

Employees are adding a corrosive call Steam Dragon to the boiler system.   2. Employees were using various spot stain cleaners such as, Pecrin, Multispot, Pyratex, and Spotless.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    On or about February 4, 2019, the employer failed to develop and implement an energy control program. Employees will add a corrosive chemical and blow down the boiler daily. Employees exposed to the unexpected release of corrosive chemical and burns to the face and eyes. The boiler had such energies as, thermal, pressure, electrical and a chemical corrosive.
