OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a struck by hazard:  On or about February 4, 2019 and times prior thereto, the structural integrity to storage racks had become compromised from: 1) unstable and rusted columns/frames, 2) footing not secured to the floor; 3) multiple dented members caused partly by forklifts; 4) broken or missing diagonal struts or braces, and 5) no plaques with legible maximum permissible unit load and/or maximum uniformly distributed load per level. Employees were exposed to a struck-by hazard from the collapse of the storage racks and/or falling products in the following locations:    a) Storage racks in the east section of the warehouse/workroom; and  b) Storage racks in the west section of the warehouse/workroom.
