29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered: Production area: On or about 02/04/2019 the Clark forklift "CGC 25" was using six foot fork extensions. The forklift was not marked to show the identity of the extensions or rated capacity while using the extensions.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: Grinding room: The two Baldor bench grinders were not equipped wo tongue guards. The right bench grinder had 2 inch openings and the left grinder had one and three quarter inch openings.
