29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  a)     On or about February 4, 2019 the employer did not ensure that employees performing blade changes on the two vertical band saws in the cut/etch department implemented energy control application steps to isolate the equipment from the hazardous energy source(s).  Employees were exposed to amputation injuries while performing blade changes.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a)     On or about January 23, 2019, the employer did not assure that unused portion of the cutting blade on the two vertical band saws in the cut/etch department was properly guarded exposing employees to amputation hazards during cutting operation.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch.   a)  On or about February 4, 2019, the employer did not ensure that the tongue guard on the  #1 Jet bench top grinder in the cut/etch department was equipped with the tongue guard positioned within ¼ inch of the abrasive wheel's peripheral edge.  Employees operating the grinder in this condition to buff ends of parts were exposed to struck-by injuries associated with breakage of the wheel.
