29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:            a) On or about February 4, 2019, employees were exposed to electrical hazards where carpentry and related work activities were conducted adjacent to an energized ComEd electrical service drop.            No abatement certification or documentation required for this violation.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):                a) On or about February 4, 2019, employees were exposed to the hazard of falling approximately 12 feet from a second story deck while performing carpentry work without fall protection.        b) On or about February 4, 2019, employees were exposed to the hazard of falling approximately 15 feet from a second story window/wall opening while performing carpentry work without fall protection.         c) On or about February 4, 2019, employees were exposed to the hazard of falling approximately 21 feet from a pitched roof deck while performing carpentry work without fall protection.       In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:           a) On or about February 4, 2019, employees were exposed to the hazard of falling approximately 10 feet where a portable extension ladder did not extend at least 3 feet above a second story deck landing surface.     b) On or about February 4, 2019, employees were exposed to the hazard of falling approximately 12 feet where a portable extension ladder did not extend at least 3 feet above a pitched roof deck landing surface.              In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
