29 CFR 1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached), where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 M) above the walking/working surface, is not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system.  On or about January 29, 2019, on the second floor of a single family dwelling where an employee was exposed to a fall hazard exceeding six feet when performing plumbing work without being protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system.

29 CFR 1904.39(a)(2): The employer failed to report the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, within (24) twenty-four hours.   On or about January 29, 2019, an employee was hospitalized while performing plumbing work without the employer reporting the work-related incident within 24 hours.
