29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: (a) On or about February 4, 2019, employee(s) were exposed to crushing/pinch point hazards while working on and around the Axor Mixer #1. The mixer was not properly locked out while clearing the excess product, which exposed employees to a rotating auger and amputation injury.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (b) On or about February 4, 2019, employee(s) were exposed to crushing/pinch point hazards while working on and around the Axor Mixer #1. The interlocks on the top guard were not working properly exposing employees to the rotating mixer/auger and amputation injury.
