OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fractures from falling:    On or about 13 February 2019, employees were exposed to fall hazards while walking and working from free turning roller conveyors to load material into the Universal Press. Employees are also required to walk across the free turning rollers to gain access to their work area. This exposed employees to injuries from falling.    Abatement documentation is not required for this item.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.    On or about 31 January 2019, the employer failed to provide adequate machine guarding to protect operator(s) and other employees from caught-in, pinch point, and struck-by hazard(s) while operating or working near all production equipment to include the Universal Press. The Universal Press has machine guarding hazards in the following areas:    a. The Cutters- No guarding was provided to prevent the operator or other employees from inadvertently contacting the rotating cutting parts of the machine during the operating cycle. This exposed employees to a caught-on hazard.     b. The Powered Feed Rollers- The employer did not provide adequate guarding to protect employees against inadvertent contact with the powered feed rollers. This exposed employees to pinch points and caught-in hazards.    Abatement documentation is not required for this item.

29 CFR 1910.219(f)(3): Sprockets and chains. All sprocket wheels and chains shall be enclosed unless they are more than seven (7) feet above the floor or platform. Where the drive extends over other machine or working areas, protection against falling shall be provided.     On or about 13 February 2019, the employer failed to properly enclose all sprockets and chains with an adequate enclosure to ensure that employees did not inadvertently come into contact with the amputation hazards on all of the production equipment to include the Universal Press. The Universal Press has the following locations of exposed chain/sprockets:    a. Conveyor- Employees are exposed to chain/sprockets when working at the conveyor end of the machine. Employees gather finished product in this area and the chain/sprocket powers the product conveyors.       Abatement documentation is not required for this item.
