29 CFR 1910.147(d)(3): Machine or equipment isolation. All energy isolating devices that are needed to control the energy to the machine or equipment shall be physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s). On or about January 15, 2019, the employer did not ensure the electrical energy isolating device (cord and plug) was operated in a manner to isolate the electrical power of a STA Packaging Taping (case sealer) machine, Model No. STA-37, Serial No. At021932, while a maintenance leadman performed service and maintenance (unjamming) operations.
