29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)  a) 7924 Pelican Reed Circle- employees in the process of plastering stucco were exposed to fall hazards up to 13' 9", in that, work platforms on the 2nd level of the scaffolding system were not fully planked or decked. Observed on or about 2/5/2019.

29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames,  or uprights did not bear on base plates, mud sills or other adequate firm foundation:  a) 7924 Pelican Reed Circle- the base of the tubular welded frame scaffolding was not adequately supported, in that, mudsills were not installed on the scaffold legs to provide a firm foundation. Observed on or about 2/5/2019.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.    a) 7924 Pelican Reed Circle- employees were exposed to fall hazards up to 13' 9" while climbing up and down the end section of a walk through style tubular welded frame scaffold. An access ladder was not installed.  Observed on or about 2/5/2019.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  a) 7924 Pelican Reed Circle- employees in the process of plastering stucco were exposed to a 13' 9" fall hazard, in that, a guard rail was not installed on the end of the top work platform, the scaffolding system, west/front side of residence.  Observed on or about 2/5/2019.

29 CFR 1926.452(c)(2): Tubular welded frames and panels were not braced by cross, horizontal, or diagonal braces, or combination thereof, which would have secured vertical members together laterally:   a) 7924 Pelican Reed Circle- employees in the process of plastering stucco were exposed to fall, collapse and struck by hazards, in that, the tubular welded frame scaffolding system (3'6" deep, 7' wide by 6'4"' high) 2 levels high by 5 columns across was missing cross bracing along the interior side on the 1st and 2nd levels.  Observed on or about 2/5/2019.
