29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: a) Exterior of Building - Roof Front: Employee without proper means of fall protection was exposed to fall hazards of approximately 30 feet while engaged in framing activities , on or about 2/5/19. b) Interior of Building - 3rd Floor: Employee without proper means of fall protection was exposed to fall hazards of approximately 8 feet while engaged in framing activities by walking over the top plates, on or about 2/5/19. c) Interior of Building - 2nd Floor: Employees utilizing a platform with missing railings were exposed to fall hazards of approximately 12 feet while engaged in framing activities, on or about 2/5/19. d) Exterior of Building - Roof Front: Employee without proper means of fall protection was exposed to fall hazards of approximately 30 feet while engaged in framing activities , on or about 2/7/19. There were no fall protection plans on site or in place.

29 CFR 1926.1052(c)(1): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail and one stairrail system along each unprotected side or edge: a) Interior of Building - 1st Floor/2nd Floor: Employees were using an interior stairway that did not have a handrail to access the 2nd floor of a residential building, exposing employees to a fall hazard of approximately 12 feet, on or about 2/5/19.
