29 CFR 1910.303(g)(1)(i)(B): For electric equipment operating at 600 volts, nominal, or less to ground, and likely to require examination, adjustment, servicing, or maintenance while energized, the working space did not permit at least a 90-degree opening of equipment doors or hinged panels:    ULTRASONIC DEPARTMENT:    On or about and at times prior to March 7, 2019, an emergency disconnect for the cranes on the northwest side of the department did not have ample working space to be able to open the door 90 degrees.

29 CFR 1910.303(g)(1)(i)(C): For electric equipment operating at 600 volts, nominal, or less to ground, and likely to require examination, adjustment, servicing, or maintenance while energized, the width of work space was not clear and did not extend from the grade, floor, or platform to the height required by paragraph (g)(1)(vi) of 29 CFR 1910.303:  PRODUCTION AREA:  On or about February 6, 2019, electrical panels in the Shipping/Receiving and Ultrasonic Departments were blocked by stored materials.

29 CFR 1904.29(b)(3): Each recordable injury or illness on the OSHA 300 Log and 301 Incident Report was not recorded within seven (7) calendar days of receiving information that a recordable injury or illness has occurred.    COMPANY-WIDE:    On or about and at times prior to February 6, 2019, the employer had not recorded on the OSHA 300 Log an injury that occurred on January 21, 2019 and resulted in restricted duty.  The injury would be appropriately captured  with a check in Column I (Restricted Duty/Transfer), 6 days restriction in Column L, and a check in Column M1.    Abatement Certification and Documentation is required.

29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:  COMPANY-WIDE:  On or about and at times prior to February 6, 2019, the employer required employees to wear N-95 particulate respirators when making chemical additions to dip tanks.  The employer had not developed and implemented a written respiratory protection program.

29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible:  COMPANY-WIDE:  On or about and at times prior to February 6, 2019, the employees who were allowed to wear N-95 particulate respirators on a voluntary basis had not been provided with training in accordance with Appendix D of this standard.
