29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: a) Oscro Warehouse, 4875 Palm Harbor Blvd - East side of the building - employees working from a Genie aerial lift, Model S-40, were exposed to an approximate 20 foot fall hazard, in that, restraint devices were not utilized, observed on or about 02/06/2019. b) Oscro Warehouse, 4875 Palm Harbor Blvd - South side of the building - employees working from a Genie aerial lift, Model S-40, were exposed to an approximate 15 foot fall hazard, in that, restraint devices were not utilized, observed on or about 02/06/2019. c) Oscro Warehouse, 4875 Palm Harbor Blvd - South side of the building - employees working from a JLG aerial lift, Model 400S, were exposed to an approximate 15 foot fall hazard, in that, restraint devices were not utilized, observed on or about 02/06/2019.
