29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates, mud sills or other adequate firm foundations:    a)  At the jobsite, on or about 2/5/19:  Employees worked from a ladder jack scaffold that was built onto a 16:12 pitch shed roof where one of those ladder uprights was not braced from slipping along the slope of the roof.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a)  At the jobsite, on or about 2/5/19:  Employees accessed a jack-plank on a 16:12 pitch roof from an extension ladder where that ladder did not extend at least 3 feet above the surface being accessed.
