On or about January 21, 2019, and at times prior thereto, at the Trinity Valley School baseball fields, the employer did not ensure employees working on top of the dugouts performing roof decking activities were protected from falling approximately 9 feet and 11 inches to the ground below.  Employees were exposed to an unprotected fall hazard.
