This violation was most recently documented at a worksite located at or near 87725 S. Hwy 7, Ainsworth, Nebraska 69210 on or about March 20, 2019. The Solberg drill press was being used by an employee without a guard. Instance b) Employees are exposed to the hazard of a pinch point. This violation was most recently documented at a worksite located at or near 87725 S. Hwy 7, Ainsworth, Nebraska 69210 on or about March 20, 2019. The Chicago Press Brake did not have guarding at the point of operation. Instance c) Employees are exposed to the hazard of a pinch point. This violation was most recently documented at a worksite located at or near 87725 S. Hwy 7, Ainsworth, Nebraska 69210 on or about March 20, 2019. The Verson Press Brake did not have guarding at the point of operation.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours, either by telephone or in person to the OSHA Area Office that is nearest to the site of incident, by telephone to the OSHA toll-free central telephone number, 1-800-321-OSHA (1-800-321-6742), or by electronic submission using the reporting application located on the OSHA public web site at www.osha.gov: The employer is failing to ensure that employees are protected from the hazard of no injury and illness identification. This violation was most recently documented at a worksite located at or near 87725 S. Hwy 7, Ainsworth, Nebraska 69210 on or about March 20, 2019. The employer did not notify the OSHA Omaha Area office of an amputation that took place on October 26, 2018 within 24 hours of its occurrence. The Omaha Area Office learned of this incident during inspection activity that took place during March 20-22, 2019. An employee sustained an amputation on October 26, 2018 when their hand was pinched between a part and a die while operating the Chicago Press Brake.
