29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  (a) New residential construction located on Lot #59, north side of the structure: On or about 02/05/2019, and at times prior, the employer exposed employees to struck-by hazards from overhead objects including sheets of plywood, in that employees working below were not provided with head protection while hoisting building materials up to the upper level.

29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  (a) New residential construction located on Lot #59, Second level: On or about 02/05/2019, and at times prior, the employer exposed employees to struck-by hazards while using pneumatic framing nail guns, in that employees were not provided with protective eyewear while operating these tools at 120 PSI engaged in framing activities.

29 CFR 1926.304(d): Portable, power-driven circular saw(s) were not equipped with a guard above and below the base plate or shoe:  (a) New residential construction located on Lot #59, Front side: On or about 2/05/2019, and at time prior, the employer exposed employees to severe laceration and/or amputation hazards while operating circular saws, in that the lower guard was fixed in the upward position with a wood shim which prevented the lower guard from retracting immediately after a cut was made.

29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees:  (a) New residential construction located on Lot #59, Ground level: On or about 02/05/2019, and at times prior, the employer failed to inspect all electrical equipment, in that circular saws had damaged strain relief, extension cords had torn and frayed casings and were not removed from service prior to the start of the shift.
