29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, was not protected by protective helmets. On or about February 6, 2019, employees were unloading roofing materials from the roof of the three story building under construction in the fall zone without head protection, exposing employees the hazard of being struck by materials or equipment.

29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction 6 feet (1.8 m) or more above lower levels were not protected from falling by the use of guardrail systems, safety net systems or personal fall arrest systems nor was an alternative fall protection plan which meets the requirements of 1926.502(k) developed and implemented: On or about February 6, 2019, and at times prior thereto, workers were conducting roofing operations on the three story apartment building approximately 32 feet high without fall protection, exposing employees to a fall hazard.
