29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    On or about 1/28/19, the employer failed to ensure that the Bridgeport Textron, Series I 24 P, Diagram #0027773, cutting tool was provided with machine guarding methods. The cycle of the Bridgeport is started with two (2) hand controls, and when the tooling lowers below the guard and comes within about an 1/8" of the product, the operator removes their hands from the controls. The cutting tool continues to spin unguarded during the cutting process. The opening from the guard to the spinning tool was about 1 1/4 ", the cutting tool was about 14.5" from the operator's hand controls, and about 10" in front of the operator.
