29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets. On the east side of the construction site, employees engaged in framing activities were working at lower levels below other workers above them were not wearing hardhats exposing them to struck-by falling tools and material.

29 CFR 1926.304(d): 29 CFR 1926.304(d): Each portable, power-driven circular saw withdrawn from the work did not automatically and instantly return the lower guard to the covering position of the saw. On the east side of the construction site, employees operating Makita skill saw with the lower guard wedged in the up position while cutting framing material were exposed to amputation hazards.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.
