29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladder(s) were not used at an angle such that the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the working length of the ladder:  a.    On or about February 6, 2019, at the left lower eave, two employees spreading tarps accessed the roof with an extension ladder that was set at 62 degrees, an angle 13 degrees shallower than required by the standard, exposing employees to an 8 feet fall hazard from the ladder.

29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section:  a.     Employees performing roofing work are not protected from fall hazards by the use of a fall protection system.  Travis Slaughter d/b/a Florida Roofing Experts fails to assure effective protections against falls as required by the standard, as demonstrated on February 6, 2019, at a job site at 2187 South Cranbrook Ave, Saint Augustine, Florida, when 6 roofing employees working on the 6:12 pitch slopes of first-story and second-story roofs were exposed to 8-feet to 22-feet fall hazards from the roof eaves and the raked edges of the peaks of roof gables in that they were not protected by a fall protection system.  Travis Slaughter d/b/a Florida Roofing Experts, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1318779, Willful citation number 1, item number 1, and was affirmed as a final order on December 14, 2018, with respect to a workplace located at 1116 Nochaway Drive, St. Augustine, FL 32092.  Great White Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1251412, Willful citation number 1, item number 1, and was affirmed as a final order on October 23, 2017, with respect to a workplace located at 1601 University Blvd. North, Jacksonville, FL 32211.  Great White Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1207836, Willful citation numbers 1 through item number 5, and was affirmed as a final order on August 24, 2017, with respect to a workplace located at 31 Millikan Lane, Saint Augustine, FL 32080. 29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a.    On or about February 6, 2019, at the job site, the roof access ladder used by 5 roofing employees at the left eave was extended only 14 inches above the edge of the eave, exposing the employee to an 8 feet fall hazard from the top of the ladder.  b.    On or about February 6, 2019, at the job site, the roof access ladder used by the Crew Leader was extended only 22 inches above the roof edge above the front entrance, exposing the Crew Leader to an 8 feet fall hazard from the top of the ladder. 29 CFR 1926.1053(b)(4): Ladders were used for purposes other than the purposes for which they were designed:  a.    On or about February 6, 2019, the Crew Leader separated the two sections of an aluminum extension ladder and used the fly-section as a roof access ladder that did not have proper base traction.
