29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladder(s) were not used at an angle such that the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the working length of the ladder: a. On or about February 6, 2019, at the left lower eave, two employees spreading tarps accessed the roof with an extension ladder that was set at 62 degrees, an angle 13 degrees shallower than required by the standard, exposing employees to an 8 feet fall hazard from the ladder.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section: a. Employees performing roofing work are not protected from fall hazards by the use of a fall protection system. Travis Slaughter d/b/a Florida Roofing Experts fails to assure effective protections against falls as required by the standard, as demonstrated on February 6, 2019, at a job site at 2187 South Cranbrook Ave, Saint Augustine, Florida, when 6 roofing employees working on the 6:12 pitch slopes of first-story and second-story roofs were exposed to 8-feet to 22-feet fall hazards from the roof eaves and the raked edges of the peaks of roof gables in that they were not protected by a fall protection system.
