Drainage was not maintained where wet process(es) were in use: On or about February 7, 2019, the walking working surfaces around the Hendrix number 12 wire machine were saturated with coolant fluid causing the floor to be wet and slippery.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about February 7, 2019, the following machines were not properly safeguarded. 1. DELETED in INFORMAL see signed ISA 2. The Redaelli #2 buncher machine Strander/Cabler/Twinner cabinet had an access door interlock assembly that was not attached to ensure the machine shut down when the access door is opened. 3. The Redaelli #3 buncher machine Strander/Cabler/Twinner cabinet access door interlock assembly was not in alignment with the pad eye opening to ensure the machine shut down when the access door is opened.
