29 CFR 1926.451(b)(1): Platform(s) on all working levels of scaffolds, were not fully planked or decked between the front uprights and guardrail supports such that the space between adjacent units, and the space between the platform and the uprights is no more than 1 inch (2.5 cm) wide:    a) West side of plaza, parking lot, on or about 2-5-19 through 2-7-19:  Employees were working on a scaffold system that was not fully planked, exposing employees to a fall hazard of 25 feet 4 inches.

29 CFR 1926.451(c)(1): Supported scaffolds with a height to base width (including outrigger supports, if used) ratio of more than four to one (4:1) were not restrained from tipping by guying, tying, bracing, or equivalent means, as set forth in subparagraphs (i) - (iii) of this paragraph:    a) West side of plaza, parking lot, on or about 2-5-19 through 2-7-19:  Employees were working on a scaffold system that was greater than a 4:1 height to width ratio, exposing employees to a fall hazard of 25 feet 4 inches.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level:    a) West side of plaza, parking lot, on or about 2-5-19 through 2-7-19:  Employees were working on a scaffold platform without the use of fall protection, exposing the employee to a fall hazard of 25 feet 4 inches.     b) West side of plaza, parking lot, on or about 2-5-19 through 2-7-19:  Employees were working on an unprotected open sided scaffold platform without the use of fall protection, exposing the employee to a fall hazard of 25 feet 4 inches.     c) West side of plaza, parking lot, on or about 2-5-19 through 2-7-19:  Employees were working on a scaffold platform with an unprotected open end of walkthrough frames without the use of fall protection, exposing the employee to a fall hazard of 25 feet 4 inches.
