29 CFR 1926.451(e)(9)(iv): The employer did not ensure that cross braces on tubular welded frame scaffolds were not used as a means of access or egress. On or about February 6, 2019, at least one employee working on a commercial building was exposed to falls of approximately 40 feet when climbing the cross braces to gain access to roof.
