29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided:    (a) South building - On or about February 8, 2019 Employees were exposed to struck by and fall hazards due to employer not providing employees with a ladder to gain access to second floor framed wall. Employees climbed up the wall bracing to get up and down from the wall that was 22 feet 8 inches high.

The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.  (a) South building - On or about February 8, 2019 Employees were exposed to struck by and fall hazards while framing a new two story building 22 feet 8 inches high without any fall protection and training to recognize hazards.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) South building - On or about February 8, 2019 Employees were exposed to struck by and fall hazards while framing the new two story building gaining access using an aluminum extension ladder that did not extend 3 feet past upper landing.
