29 CFR 1910.215(a)(4): Grinding machinery was not used with work rest(s) to support offhand grinding work: On or about 2/11/2019, at maintenance shop of Five Star Gourmet Food of Florida, Inc., 3600 Shaw Blvd, Naples, FL 34117, an employee was exposed to a struck by hazard while working with a bench grinder (i.e. Ryobi, BHG616) that lacked a work rest on its left side.

29 CFR 1910.1200(h)(2)(iii): The employer did not provide information to the employees as to the location and availability of the written hazard communication program, and material safety data sheets required by 29 CFR 1910.1200: On or about 6/12/2019, at Five Star Gourmet Food of Florida Inc., 3600 Shaw Blvd, Naples, FL 34117, employees were exposed to hazardous chemicals including but not limited to, bleach, nitrogen, FS Process Clear (a corrosive, with a pH of 13.0-13.5), Perox (corrosive, with a pH less than 1), while working in production, sanitation and cleaning operations, as they were not knowledgeable of the location and availability of the company's hazard communication program and/or safety data sheets.
