29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, service rooms, and walking-working surfaces were not kept in a clean, orderly, and sanitary condition: On or about 02/11/2019, Natures Bakery, located at 8860 Pershall Road, Hazelwood, MO, had employees exposed to the hazards located at or near their working stations. The walking working surface (concrete floor) had various chairs, pallets, blue bin containers and fig bars on the floor at or near work station(s) and walkways. This initiated an incident which caused an employee to fall and fracture their ankle.

It had been over 1.5 years and in some cases over 2 years since a review had been performed within the Maintenance Department and their personnel.
