29 CFR 1926.451(a)(6): Scaffolds were not designed by a qualified person. On or about 11 February 2019, and at times prior thereto, at the Mountain Ranch Apartments, 2425 East Riverside Drive, Austin, TX 78741, at least three workers were raised or lowered by a job-site made, wooden trash box supported by the forks of a rough terrain forklift JLG 10054. The trash box was elevated to the roof levels of three-story apartment buildings being reroofed exposing the workers to the hazard of falling from a height of approximately 29 feet.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b). On or about 11 February 2019, and at times prior thereto, six roofers were on the rooftop of a three-story apartment building without fall protection, exposing the workers to a fall hazard of approximately 29 feet to the ground below.
