29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles: (a) New residential construction located on Lot #58, Roof trusses: On or about 02/05/2019, and at times prior, the employer exposed employees to struck-by hazards which include plastic debris and 3-inch fasteners while operating pneumatic nail guns, in that protective eyewear was not provided to employees engaged in framing activities using these tools at 120 PSI.

29 CFR 1926.304(d): Portable, power-driven circular saw(s) were not equipped with a guard above and below the base plate or shoe: (a) New residential construction located on Lot #58, Front side of structure: On or about 02/05/2019, and at times prior, the employer exposed employees to severe laceration and amputation hazards while operating a circular saw, in that the lower guard failed to retract immediately after a cut was made because a wood shim was used to keep the lower guard in the fixed upward position which exposed the blade under the base plate of the circular saw.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: (a) New residential construction located on Lot #58, Roof trusses and top plate: On or about 02/05/2019, and at times prior, the employer exposed employees to fall hazards over 9-feet, 6-inches from the lower level, in that a fall protection system was not provided to employees engaged in the installation of 10/12 pitched roof truss framing activities.

29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees: (a) New residential construction located on Lot #58, Garage and concrete slab area: On or about 02/05/2019, and at times prior, the employer failed to inspect all the electrical cords prior to the start of the work shift. A 50-ft. extension cord used to power the Porter Cable Air Compressor had a torn casing and damaged strain relief and was not repaired or removed from service.
