29 CFR 1926.405(a)(2)(ii)(I): Flexible cords and cables used for temporary wiring were not protected from damage:    On or about February 8, 2019, at the jobsite located adjacent to 10304 NW 9th Street Circle, Miami, Florida 33072, an orange 110-120 volt electrical flexible extension cord was extended across a parking lot in such a way that vehicular traffic running over could damage the cord and cause an employees to suffer electrocution when handling a damaged cord.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    On or about February 8, 2019, at the jobsite located adjacent to 10304 NW 9th Street Circle, Miami, Florida 33072, employees were accessing a roof from a Gorilla Ladder MPX 17 multi-task ladder when the side rails of the ladder did not extend 3 feet above the landing surface of the roof.

On or about February 8, 2019, at the jobsite located adjacent to 10304 NW 9th Street Circle, Miami, Florida 33072, employees were engaged in tearing off an existing roof and exposed to a fall hazard of a leading edge of 9.5 feet (113 inches)above the ground below without means the of a fall protection system.    The M.G. Excellent Services Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1230010, citation number 01, item number 01, and was affirmed as a final order on 06/27/2017, with respect to a workplace located at 2801 NW 103rd Street Miami, FL 33147.

29 CFR 1926.503(a)(1): A training program was not provided to each employee who might be exposed to fall hazards, to enable that employee to recognize the hazards of falling, and be trained in the procedures to be followed to minimize these hazards:    On or about February 8, 2019, at the jobsite located adjacent to 10304 NW 9th Street Circle, Miami, Florida 33072, the employer did not provide employees exposed to potential fall hazards with training on the recognition and identification of the hazards of falling, as well as, procedures to be followed in order to minimize these hazards. Untrained employees were exposed to a fall hazard of 9.5-feet during asphaltic shingles removal.    The M.G. Excellent Services Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1230010, citation number 01, item number 02, and was affirmed as a final order on 06/27/2017, with respect to a workplace located at 2801 NW 103rd Street, Miami, FL 33147.
