29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by this standard, the employer did not ensure that live parts of electric equipment operating at 50 volts or more were guarded against accidental contact by use of approved cabinets or other forms of approved enclosures:    (a) Beal Distributing, Inc., 4815 North Northview Avenue, Sioux Falls, SD 57107:  On or about and at times prior to February 12, 2019, the employer did not ensure knockouts were replaced on the top of a light switch in the old phone room area.  This hazard could cause an employee to come into contact with live power    (b) Beal Distributing, Inc., 4815 North Northview Avenue, Sioux Falls, SD 57107:  On or about and at times prior to February 12, 2019, the employer did not ensure live parts of an outlet on the east side of the warehouse were protected in that a cover was broken exposing employees to a shock hazard.

One spray bottle with adhesion promoter was unlabeled and another orange juice container with an unknown substance were found.

29 CFR 1910.1200(h)(1): The employer did not provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new chemical hazard the employees have not previously been trained about is introduced into their work area:    (a) Beal Distributing, Inc., at 4815 North Northview Avenue, Sioux Falls, SD 57107:  On or about and at times prior to February 12, 2019, the employer did not ensure employees in the print shop and loading dock were trained to understand the chemicals which they were exposed to, including personal protective measures and proper storage and handling in that employees were using chemicals including acetone, isopropyl alcohol and adhesion promoter without a clear understanding of the properties of each chemical such as PPE, storage, labeling, and SDS locations.

29 CFR 1910.37(a)(3): The employer did not ensure that exit routes were free and unobstructed:   a)Beal Distributing, Inc., 4815 North Northview Avenue, Sioux Falls, SD 57107:  On or about and at times prior to February 12, 2019, the employer allowed pallets and a floor scrubbing machine to block the marked exit route between the loading dock area and the main hallway of the building.    Abatement Note: 1. No materials or equipment may be placed, either permanently or temporarily, within the exit route.  2. The exit access must not go through a room that can be locked, such as a bathroom, to reach an exit or exit discharge, nor may it lead into a dead-end corridor. Stairs or a ramp must be provided where the exit route is not substantially level.
