29 CFR 1926.452(k)(5): Scaffold platforms were bridged on to another: a) On or about 02/11/19, on the roofing jobsite at 94 Hartwell Road, Buffalo, NY: Ladder Jack Scaffolding was set up on the west side of the house. An aluminum pik overlapped (bridged ) the gap between the piks on the scaffolding. NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) On or about 02/11/19, on the roof on the west side of a house at 94 Hartwell Road, Buffalo, NY: Two employees engaged in roofing work on a steep roof were exposed to a fall of approx. 20 ft. No fall protection system was being used. Pitch of the roof was approx. 9 in 12. NO ABATEMENT CERTIFICATION REQUIRED
