On 12/19/2018, employees of Victor Bonilla were installing a new roof on residential buildings at 701-705 Hyde Drive, OFallon, Missouri. Eye protection was not used by employees involved with using pneumatic nail guns during roofing operations.

Employees engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest systems. On 12/19/2018, employees of Victor Bonilla were installing a new roof on residential buildings at 701-705 Hyde Drive, OFallon, Missouri. Employees were installing shingles approximately 21 feet above a lower level. Employees were not utilizing guardrail systems, a safety net system, or a personal fall arrest system. The employees were observed on the roof of the building without fall protection exposing themselves to a fall to the ground below.

The program did not enable each employee to recognize the hazards of falling and did not train each employee in the procedures to be followed in order to minimize these hazards. On 12/19/2018, employees of Victor Bonilla were installing a new roof on residential buildings at 701-705 Hyde Drive, OFallon, Missouri. The employees were not able to recognize fall hazards and the procedures necessary to reduce or eliminate these hazards.
